Download, print, and complete the following three forms. All three must be signed and uploaded below along with your registration.

Step 2: Additional Requirements

In addition to the three forms above, California requires the following before you can begin clinical training. It’s okay if you don’t have these ready yet — you can bring or upload them later, but your file won’t be complete until we have them.

Health Examination Form – download and take to your physician for a physical exam
Live Scan / DOJ Background Check Request (BCIA 8016) – download and bring to a Live Scan location
Here’s the guide for the live scan
Record of Tuberculin (TB) Testing or Chest X-Ray – download and have your provider complete this after testing
BLS/CPR Certificate – upload a copy below if you already have one
